The ultimate nail in the coffin is that the average American gets much less out of the 12K spent on insurance than they put in. although its higher than it was in 2002, its likely less than $1000/person for median spent (50th percentile). While we can talk about fairness and social equity till we're blue in the face, when people don't see value, they tend to vote with their feet.
